I . All personnel who received the 2026 Medical Allowance are required to submit
their proof of HMO availment on or before November 20, 2026.
2. Please submit the fouowing documents:
a. Copy of HMO Agreement/ Contract
b. Vand HMO ID Card
c. Official Receipt
3. Failure to submit within the prescribed period may affect future releases of
simflar benefits.
